HIPAA Notice of Privacy Practices

Notice of Privacy Practices

Effective Date: January 1, 2024. This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

We are required by law to maintain the privacy of your protected health information (PHI), to provide you with this notice of our legal duties and privacy practices, and to notify you following a breach of unsecured PHI. We are required to abide by the terms of this notice currently in effect.

How We May Use and Disclose Your Health Information

Treatment

We may use and disclose your PHI to provide, coordinate, or manage your healthcare and related services. For example, we may share your prescription information with your prescribing physician to coordinate your care.

Payment

We may use and disclose your PHI to obtain payment for services we provide to you. For example, we may submit claims to your insurance company that include information about your prescriptions.

Healthcare Operations

We may use and disclose your PHI for our healthcare operations, including quality assessment, training, licensing, and business management activities necessary to operate our pharmacy.

As Required by Law

We will disclose your PHI when required to do so by federal, state, or local law, including to public health authorities, law enforcement, or in response to a court order or subpoena.

Business Associates

We may share your PHI with third-party "business associates" who perform services on our behalf (e.g., billing services, IT providers). We require all business associates to protect your PHI under a written agreement.

Your Rights Regarding Your Health Information

Right to Access

You have the right to inspect and obtain a copy of your PHI that we maintain. Submit a written request to our Privacy Officer. We will respond within 30 days.

Right to Amend

You have the right to request that we amend your PHI if you believe it is incorrect or incomplete. We may deny your request under certain circumstances.

Right to an Accounting of Disclosures

You have the right to request a list of certain disclosures we have made of your PHI for purposes other than treatment, payment, or healthcare operations.

Right to Request Restrictions

You have the right to request restrictions on how we use or disclose your PHI. We are not required to agree to your request, except when you request we not disclose PHI to your health plan for services you paid for in full.

Right to Confidential Communications

You have the right to request that we communicate with you about your PHI in a certain way or at a certain location (e.g., only by mail to a specific address).

Right to a Paper Copy

You have the right to receive a paper copy of this notice upon request, even if you have agreed to receive it electronically.

Our Responsibilities

Safeguarding Your Information

We maintain physical, electronic, and procedural safeguards to protect your PHI. All staff are trained on HIPAA privacy and security requirements.

Breach Notification

We will notify you promptly if a breach of your unsecured PHI occurs that may compromise the privacy or security of your information.

Changes to This Notice

We reserve the right to change this notice and to make the revised notice effective for PHI we already have about you. We will post the current notice in our pharmacy and on our website.

Privacy Policy

Effective Date: January 1, 2024. This policy explains how RX Box Pharmacy collects, uses, and protects your personal information when you use our website and services.

1. Information We Collect

We collect information you provide directly to us, including: name, date of birth, contact information (phone, email, mailing address), prescription and medication information, insurance and billing information, and health-related information necessary to fill your prescriptions. We also collect limited technical data when you use our website, such as IP address, browser type, and pages visited, through standard server logs and cookies.

2. How We Use Your Information

We use your information to: fill and manage your prescriptions; verify your identity; process insurance claims and payments; communicate with you about your orders, refills, and health; comply with legal and regulatory obligations; improve our website and services; and send you important notices about your medications or our services. We do not use your health information for advertising or sell it to third parties.

3. How We Share Your Information

We share your information only as necessary to provide pharmacy services: with your prescribing healthcare providers to coordinate care; with insurance companies and pharmacy benefit managers to process claims; with business associates (e.g., delivery partners, IT vendors) under HIPAA-compliant agreements; and as required by law. We do not sell, rent, or trade your personal or health information.

4. Cookies and Website Tracking

Our website uses essential cookies to maintain session state and security. We may use analytics tools to understand aggregate usage patterns. We do not use tracking cookies for advertising. You may disable cookies in your browser settings; however, some features of our website may not function properly without them.

5. Data Security

We implement industry-standard physical, administrative, and technical safeguards to protect your information, including SSL/TLS encryption for data in transit, access controls, and regular security assessments. No method of transmission over the internet is 100% secure; however, we take all reasonable steps to protect your data.

6. Data Retention

We retain your prescription and health records as required by applicable state and federal law (typically a minimum of 5–10 years). Website usage data is retained for up to 24 months. You may request deletion of non-health data by contacting us, subject to our legal retention obligations.

7. Your Choices

You may opt out of non-essential marketing communications at any time by contacting us or using the unsubscribe link in our emails. You may request access to, correction of, or deletion of your personal information (subject to legal retention requirements) by contacting our Privacy Officer.

8. Children's Privacy

Our online services are not directed to children under 13. We do not knowingly collect personal information from children under 13 without verifiable parental consent. If you believe we have inadvertently collected such information, please contact us immediately.

9. Changes to This Policy

We may update this Privacy Policy from time to time. We will notify you of material changes by posting the updated policy on our website with a revised effective date. Your continued use of our services after such changes constitutes acceptance of the updated policy.
